Before your first visit
Patients require a physician referral for physical therapy treatment. Then the patient or doctor's office calls DPT and we set up an appointment for evaluation. The patient should bring with them an identification card (license, school id, photo id) and proof of insurance coverage card(s). The patient will be required to fill out preliminary information about personal history, medical history including current medications and consent forms in order to begin treatment. After filling out forms patient will be given the financial policy drawn up from their insurance contract alerting them to what their financial responsibilities will be, whether in form of co pay, coinsurance or deductible.
